HASSLE-FREE CLAIMS PROCESS

There are three types of claims that you can file depending on your policy’s benefit – Accident, Illness, or Death.
Choose the type of claim you would like to file and fill out the form under each section.
You will be asked about your basic information as well as some details about the type of claim you are filing.

Step 2:
Submit your requirements.

Please secure and upload clear copies of the necessary requirements for each type of claim.
You will also be asked in this section how you would like to receive your pay-out if your request is approved.

Step 3:
Get your reference number.

Once you submit your requirements, your request will be reviewed and evaluated by our claims team. You can expect to receive an update from us via SMS and e-Mail within 7-10 working days.
You will receive a reference number that you can use to track the status of your request.

3. Police or Narration Report
4. Official Receipts (OR)

ILLNESS

We're sorry to hear that you've been unwell but we're here to help cover for your medical expenses. To get started, please prepare clear copies of the requirements below:

1. Valid Government ID
2. Attending Physician's Statement (APS)
3. Hospital Statement of Account (SOA)

DEATH

We would like to extend our deepest sympathies for your loss. Nothing can be harder than losing someone close to us, that's why we're here to help you in this trying time. To get started, please prepare clear copies of the requirements below:

1. For the Insured:
   • Death Certificate of the Deceased
   • Police Investigation Report (If the cause of death is due to an accident)
2. For the Beneficiary
   • Valid Government ID
   • Marriage Contract (If the Beneficiary is the spouse)
   • Birth Certificate (If the Beneficiary is below 18 years old)
